CREW MEMBER ENTRY FORM

Each member is required to fill out this form individually. Each member must upload a photo of his/her PASSPORT or ANY GOVERNMENT ID W/ BIRTHDAY(e.g. National ID, Driver's License). If the member has no passport or any government ID w/ birthday, he/she must upload a photo of his/her I.D. and BIRTH CERTIFICATE. It is required that the member submits BOTH his/her I.D. and BIRTH CERTIFICATE in the absence of a passport or government ID. Each member will also be required to sign the WAIVER FORM. In case he/she is a minor, his/her parent must sign instead.
