This is your official registration form. The following will be submitted through this form:

✔️ Crew Leader Information & Contact Details

✔️ Team Assistant / Assitant Coach Name (except for Minicrew)

✔️ Crew Members' Full Name & Birthday

✔️ Registration Fee Deposit (non-refundable)

Upload a photo of your deposit slip or a screenshot of your transfer. Deadline for the registration fee balance is  MARCH 31, 2026 (TUESDAY). Registration fee is inclusive of audience access to both the Semis and Finals for ALL REGISTERED DANCERS. Whether you are a finalist or not, you can watch the Semi Finals and Finals for FREE. 

Registration Fee: Php 1,500 per dancer

Sample Computation:

5 members: Php 1,500 x 5 = Php 7,500

Deposit: Php 3,000

Balance: Php 4,500

Each crew member should fill out the Crew Member Entry Form individually. Here, a photo of the member's Passport or Government I.D. containing birthday (ex. driver's license) will be uploaded. In case the member has no passport or government I.D., he/she can upload a photo of both his/her I.D. and Birth Certificate. A waiver form will also be signed through the Member Entry Form. This should be submitted by April 15, 2026 (WEDNESDAY).

Upload your music along with your crew logo & write up. Make sure to read the RULES & REGULATIONS to prevent disqualification or any deductions. Deadline of submission is on MAY 1, 2026 at 11:59 PM (FRIDAY) . Music changes will be permitted as follows:

• Semi Finals - until May 7 at 11:59PM
• Finals - until May 10 at 1:00AM
